APPOINTMENTS
To schedule an appointment, please call 715-355-1616. Our staff will be pleased to assist you in any way possible, and all efforts will be made to accommodate your schedule. A credit card is required to reserve your appointment. Please arrive 10 minutes prior to your scheduled appointment time.
For your convenience, you may download New Patient Forms to fill out prior to your first appointment. Otherwise, the forms are available to complete once you arrive.
LATE ARRIVALS
Late arrival will lessen your treatment time. In fairness to others, your treatment will end on time so the next patient's session can begin on time. With some treatments, it may be necessary to reschedule.
Appointment reminder calls are made a day or two before as a courtesy to our patients, but it is the responsibility of the patient to remember their appointment.
EMERGENCIES
Never hesitate to call 911 if you are experiencing a life threatening emergency.
For urgencies, call our office at (715) 355-1616 Tuesday-Friday 10am-6pm.
For after-hour urgencies or concerns, Dr. Bucan can be reached at 715-302-0111.
CANCELLATIONS
Your appointment is reserved exclusively for you. We believe that your time is as important to you as ours is to us. If you are unable to keep an appointment, we ask that you kindly provide at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another client.
There is a $75 charge for any cancellation less than 24 hours prior to a scheduled appointment, including complementary consultations and follow-up appointments.
NO-SHOWS
Patients who do not show up for their appointment will be charged $75. If the no-show was for a package treatment, the treatment session will be taken from your package. The $75 fee is necessary to cover expenses and valuable time allocated to patients and our technicians when the appointment is made.
CHILDREN
Children under the age of 18 need parental consent prior to consultations & treatments.
Children under the age of 12 are not allowed in our treatment rooms. To ensure the safety of children and the enjoyment of all our clients, we ask that parents or guardians make other arrangements for children while receiving services, including consultations.
PAYMENT FOR SERVICES
We accept Visa, Mastercard, Discover, debit cards, cash, & checks.
Gratuities are never expected, but greatly appreciated by our Esthetician.
We do not allow tipping for medical services or visits with our medical practitioner.
QUOTED PRICES
Prices quoted are valid for a period of 6 months from the date of the consult. Quoted prices cannot be combined with other specials or promotions.
CHECKS WITH NON-SUFFICIENT FUNDS
A $35 fee will be charged for checks returned for insufficient funds.
ACNE FOLLOW-UP APPOINTMENTS
A $45 office visit fee applies to all acne follow-up appointments.
REFUNDS
Retail Products: No refunds or exchanges on retail products is permitted unless an adverse reaction occurs.
Adverse reactions must be reported to our office within 2 weeks and documented by our physician.
According to federal law, prescription products (Rxn.) such as Latisse, Tretinoin & Hydroquinone are not returnable for any reason, including adverse reaction.
Services: Cosmetic procedures are not guaranteed to have the expected or anticipated results. Payment is for the treatment and not for specific results. Payments are not refundable, exchangeable or transferable.
